Send a Referral
After the referral is received, the service coordinator will contact the parent/guardian to gather more information about the child and family.
Evaluation & Determining Eligibility
A developmental evaluation will take place in your child's natural environment to determine program eligibility. A child is eligible for services if he or she:
- Demonstrates a significant delay (25% behind) on at least one area of development:
- Communication (understanding and use of language).
- Motor (movement and physical development).
- Cognitive (learning, thinking and problem solving).
- Self-Help (feeding, dressing, toileting and sleeping).
- Social/Emotional (forming relationships, understanding and managing emotions).
OR
- Has a diagnosis or medical condition that is likely to lead to delayed development.
Creation of an Individual Plan
For each eligible child, a meeting will be held to develop the child's Individualized Family Service Plan (IFSP), which includes the parents' / caregivers' goals and a plan for services and support from the program.
Ongoing Support Services
The Individual Family Service Plan (IFSP) will include the designation of a provider(s) who will work with your child and family; however, a full team of professionals is available to provide support whenever necessary. These include:
- Service Coordination
- Speech/Language Pathology
- Early Childhood Special Education
- Occupational Therapy
- Physical Therapy
The Birth to 3 team will provide your family with ideas and strategies to help promote your child's development and participate in activities more independently. They will focus on what is naturally happening in your child's life and provide services in your home, your child care setting and / or other places in the community. Success is measured by your child learning new skills and your family gaining confidence in meeting your child's needs.
